As with any medical practice, we need to collect certain information about you in order to serve you best. We understand this is a difficult, redundant part of visiting a physician’s office these days, but assure you it is imperative we have the correct information prior to being able to see you as a patient. If you have this information available upon arrival, the physician will be able to see you and all subsequent patients in a more timely manner.
To access your medical records online or ask your provider a non-urgent medical question, please use our new patient portal. You can register your account with the PIN number provided to you at your last visit. To request a new PIN number, please call our office at 404-355-1285.

Prescription Refills & Medication Questions
If you are ill or have any questions regarding various medications please call the main office number, (404) 355-1285, and ask for the triage nurse. Please have the following information available:
- Patient’s Name
- Patient’s Date of Birth
- Physician’s Name
- Name of Medication
If you need a prescription to be refilled, please call your pharmacy directly.
We participate with many of the HMO, PPO, and POS plans, as well as other managed care medical plans offered in this area. We also participate with Medicare. We are not participating providers for any Medicaid plans.
